To enter, all you have to do is FILL OUT THIS FORM by Sunday, April 28, at Noon EST. One winner will be selected at random from all eligible entries on Monday, April 29. The winner will be notified by email and will have 48 hours to claim their prize. if the prize is not claimed within 48 hours, an alternate winner will be chosen.
